Sports Physicals for Student-Athletes

Only $10 (Cash Only, No Insurance Needed) All proceeds are donated to the athlete’s athletic program!

Athletes should visit www.aiaonline.org/about/forms to complete their portion of the paperwork before arriving. Please fill out two forms: Form 15.7-A and Form 15.7-B.

Minors must be accompanied by a parent/guardian. Bring glasses or contacts for vision testing and wear easy to remove shoes.
